- Current and valid CNA license in Pennsylvania.
- Experience in home healthcare is preferred but not required.
- Strong clinical and interpersonal skills.
- Compassionate, patient, and attentive.
- Ability to assist with mobility and transfers (up to 50 lbs).
- Valid driver’s license and reliable transportation as needed.
- Successful completion of background check and drug screening.
- Assist clients with daily living activities (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
- Collaborate with the care team to develop and implement individualized care plans.
- Monitor and document vital signs (temperature, pulse, respiration, blood pressure).
- Assist with medication reminders and basic wound care as instructed by a registered nurse (RN).
- Provide companionship and foster a positive environment.
- Observe and report any changes in the client’s condition, behavior, or surroundings.
- Perform light housekeeping tasks to maintain a safe, clean environment.
- Adhere to company policies, state regulations, and professional standards.
